TITLE: Independent Living Coordinator – Fox Valley

DESCRIPTION / SUMMARY:
This individual is responsible for the provision of service coordination to assigned consumers of Options as well as other designated responsibilities. This position is located at Options Fox Valley Office- located at the Thompson Center on Lourdes. This position will provide services in the following counties: Fond du Lac, Green Lake, Marquette, Outagamie, Winnebago, Waupaca and Waushara.

Qualifications: Education: Bachelor's Degree in Social Work/Rehabilitation or Certified Occupational Therapist Assistant preferred. An equivalent combination of education and experience in providing services to person with disabilities may be substituted for college degree.

Experience: Knowledge of human services programs affecting people with disabilities. Knowledge of disability rights and IL philosophy. Ability to develop and provide independent living skills training. Individual with a disability preferred.

License: Valid Driver's license and insured vehicle or ability to arrange for independent travel.

JOB RESPONSIBILITIES

Essential Functions

  • Provide services to people with disabilities, including but not limited to advocacy, IL skills training, peer support (if a person with a disability), transition, and basic assistive technology assistance.
  • Provide information and referral on various disability-related subjects.
  • Provide Self-Advocacy, Skills to Pay the Bills, and other curriculum training to students/youth with disabilities.
  • Provide community activity services in Options service area to include disability related presentations, booths at area disability related fairs/events, etc.
  • Represent the interests of people with disabilities by serving as a member of area committees and boards, e.g. C-Cots, Transportation, Housing, etc.
  • Record all data as required.
  • Practice and promote the Independent Living philosophy in all aspects of this position.
  • Assist in promotion and utilization of accessible showcase model home and office complex for education of people with disabilities and general public about assistive technology and universal design.
  • Practice and promote good safety procedure.

Additional Functions

  • Act as a team member in performing other assigned duties and meetings for the efficient operation of Options.
  • Other duties as assigned by Supervisor.

SALARY: Dependent on experience and education.

40 hours per week generally 8 :00 AM – 4:30 PM however there will be occasional evening and weekend commitments.
