TITLE: Care Coordinator, Personal Care Services


The Personal Care Services Support Specialist will work with other staff to help people with disabilities receive the services that they need to live in the community. This individual will interview, hire, train and maintain personnel records and timesheets for the Personal Care Workers (PCWs). They will ensure eligibility for referred consumers to the Personal Care Program and provide other support services to the medical staff.


Education: Associate's degree or relevant professional experience.


Demonstrated hiring, orientation, training and personnel record management skills

Ability to communicate effectively orally and in written form

Experience in home health or other medical services preferred

Ability to work with a team

Knowledge and ability to use a computer

Experience in working with people with disabilities and commitment to “consumer choice”


Options for Independent Living maintains company vehicles. However, there may be times when team members need to use their own vehicles. Mileage is reimbursed at rates set by the U.S. Internal Revenue Service. The person hired for this position must have vehicle with proof of insurance and valid driver's license or arrange independent travel.

Hours: This is a 40-hour per week position. Office hours are typically 8:00 a.m. – 4:30 p.m. However, flexibility may be needed to meet the needs of consumers and PCW staff.


Essential Functions

1. Personal Care program operations

a)Contact consumer referrals and determine initial eligibility for the Personal Care Program

b)Mail and track PCW applications

c)Perform Background checks

d)Interview Personal Care Worker (PCW) candidates, make employment offers, and process personnel actions

e)Coordinate and provide PCW orientation and training including information about policies, job duties, safety and wages

f)Requesting, tracking and obtaining Medical Records and physicians orders

g)Mailing care plans developed by RN and tracking for returned signatures

h)Maintain PCW personnel records

i)PCW timesheet review, calling PCW with errors or late submissions and tracking resolutions. Logging timesheets received

j)Collaborate with administrative team to develop and carry out policies, procedures and newsletter

k)Document and follow up on all program quality concerns/complaints

l)Unemployment verification and correspondence

m) Ordering supplies as needed

n)Other tasks as may be required

2. Employ professional judgment and continue professional development

a) Ensure that professionalism is practiced in all modes of communication

b) Participate in staff meetings, workgroups, and committees

c)Identify and participate in trainings to advance knowledge and skills

3. Demonstrate Independent Living Philosophy in daily work action

a)Advocate for consumer choice and consumer-driven services

b)Protect consumer and agency information by adhering to confidentiality standards
